Registered Dietician (RD) Position Summary

The Registered Dietician comprehends and convey the best practice and keen knowledge in the medical nutrition therapy field.  Has the ability to design, implement and evaluate programs and services that is incorporated in primary care and medical nutrition therapy.


Essential Job Functions/Responsibilities

  • Plans regular and modified diets for age patients as prescribed by Medical Staff.
  • Visits and counsel patients with nutritional needs.
  • Participates in hospital, patient, and community education and outpatient programs.
  • Assist with the maintenance of the department’s Quality Assessment & Quality Improvement plan.



  • Three years of experience in a clinical setting is preferred.
  • Be registered and licensed by the State in which the facility is located.
  • Possess a B.S. degree in Food/Nutrition and/or Dietetics.
  • Be registration eligible through the American Dietetic Association, is a plus .
  • Rehabilitation experience is a plus.

RN – Case Manager

Location:  Spartanburg

Position Summary

The registered nurse (RN) plans, organizes and directs hospice care and is experienced in nursing, with emphasis on community health education/experience.  The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities.


Essential Job Functions/Responsibilities

  • Acts as Case Manager when assigned by Clinical Director and assumes responsibility to coordinate patient care for assigned caseload.
  • Completes an initial, comprehensive and ongoing comprehensive assessment of patient and family to determine hospice needs.  Provides a complete physical assessment and history of current and previous illness (es).
  • Provides professional nursing care by utilizing all elements of nursing process.
  • Assesses and evaluates patient’s status by:
    •  Writing and initiating plan of care
    • Regularly re-evaluating patient and family/caregiver needs
    • Participating in revising the plan of care as necessary
  • Initiates the plan of care in accordance with current professional standards and practice, and makes necessary revisions as patient status and needs change.
  • Uses health assessment data to determine nursing diagnosis.
  • Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions.  Includes the patient and the family in the planning process.
  • Initiates appropriate preventive and rehabilitative nursing procedures.  Administers medications and treatments as prescribed by the physician in the physician’s plan of care.
  • Counsels the patient and family in meeting nursing and related needs.
  • Provides health care instructions to the patient as appropriate per assessment and plan.
  • Assists the patient with the activities of daily living and facilitates the patient’s efforts toward self-sufficiency and optional comfort care.



  • 2 years direct patient care, previous home care or hospice experience preferred.
  • Current RN licensure and CPR required.
  • Ability to interact with individuals of diverse backgrounds and at all levels of authority.
  • Ability to deal with crisis situations and make independent judgments.
  • Ability to prioritize and organize.
  • Excellent verbal, written communication skills and strong knowledge of electronic medical records.
  • Knowledge and skills necessary to modify care according to patient’s age. Ability to interpret information to identify each patient’s requirements for care relative to his/her age specific needs.
  • Must be flexible, versatile and adaptable to change, with good critical thinking, problem solving and diplomacy skills.
  • Knowledge of procedures and nursing standards of practice, both general and specific to the specialties of medical-surgical nursing.

Working with Providence:

Providence Nurse Practitioners (NP) see 6 patients a day and conduct physical assessments, perform and interpret diagnostic tests, diagnose and treat common conditions, provide screenings, interventions, counseling, and prescribe medications as needed to patients in the geriatric population.
